The following forms are required regardless of whether you use Online Registration or fill out a paper version. Please click each option below to access the forms.
Parents unable to use Online Registration will need to complete all of the forms for each student. Along with the forms above, you will need to complete the Registration Form below for each student. If you are registering a Kindergarten student you will also need to complete the KG Questionnaire. Once you complete all required forms you can email these forms to your neighborhood school along with proof of birth and proof of address (residency).
