The Health Office is closed for summer! For any immediate needs during the summer, please contact the TCMS Main Office at 242-5100 ext. 3800.
If you need to submit updated health records, they can be sent:
- Via fax at (585) 242-7367. If there are difficulties with the Health Office fax for any reason, documents can be faxed to the TCMS Main Office at 242-2540.

6th Grade Families:
We're so excited to welcome your family to Twelve Corners in the Fall!! NYS requires all 6th graders to receive the Tdap vaccine once they turn 11-years-old (this is different from the Dtap vaccine received earlier in childhood). Please note: the minimum age for a dose to fulfill the 6th grade requirement is 10 years old. If a dose of Tdap was given before age 10, it cannot be counted towards the NYS 6th grade requirement.
New York state only allows a 14-day grace period after the start of the school year to receive this vaccine. For your child to continue participating in school after September 20th, your child needs to receive the Tdap vaccine with documentation submitted to the TCMS Health Office.
If your child turns 11 before the first day of school and has received Tdap, please send a copy of their immunization records to the TCMS Health Office.
If your child has not turned 11 years old yet, they are not "overdue" for the vaccine until their 11th birthday. Please note that your child can receive the Tdap vaccine once they turn 10 years old and are only given a 14-day grace period to participate in school following their 11th birthday. If your child turns 11-years-old after the first day of school and has not received the Tdap, please contact their healthcare provider and let the TCMS Health Office know of your child's appointment date.
7th Grade Families:
New York State requires that students entering 7th grade receive the meningococcal vaccine. If your child has already received the meningococcal vaccine, please fax a copy of their immunization records to the TCMS Health Office at 242-7367 or email them to kelsey_hadzima@bscd.org or amanda_weinman@bcsd.org. If your child hasn't received the meningococcal vaccine yet, please let us know your child's appointment date. Please note: the minimum age for a dose to fulfill the 7th grade requirement is 10 years old. If a dose of meningococcal vaccine was given before age 10, it cannot be counted towards the NYS 7th grade requirement.

7th graders will also be required by NYS to have a physical dated 9/1/2022 or more on file at school for the 2023-2024 school year. The physical must be documented on the NYS School Health Examination form.
8th Grade Families:
There are no additional NYS-mandated immunizations required in 8th grade. NYS does not require an updated physical in 8th grade, unless a student is participating in school sports.
Sports:
For information about Fall sports registration for 7th and 8th grade students (there are no sports offered at the 6th grade level), please visit the BCSD Athletics Webpage here. Students must register on rSchool and need to have a physical on file dated 9/1/2022 or more recently for Fall modified sports (for football, physicals must be dated 8/1/2022 or more recent). Modified football begins August 28th and all other modified sports begin September 6th.
Medications:
If your student will have any medication at school, please contact the TCMS Health Office. Nurses may administer medication, such as daily medications, OTC medications, or emergency medications (i.e. EpiPen, inhaler), during the school day with a written medication order from your child’s healthcare provider on file. New medication orders are required each school year. Medication orders can be faxed to the TCMS Health Office at (585) 242-7367.
All medication must be brought in and picked up by an adult. Medication must be dropped off in the original container. If half pill dosage is required, please send pill to school cut in half.
