Post-Observation Form: use the online My Learning Plan Template
The Word Doc file has been removed. Use the online My Learning Plan template to complete the Reflection write-up portion of the observation process. This is accessible once you login to the My Learning Plan site and select the appropriate observation (Announced or Unannounced). Please make sure you click "Submit" when the reflection template has been completed and ready to share. You also have the option to "Save and Finish Later", but you must click "Submit" in order for it to be shared with the evaluator.
