Section 504 is a nondiscrimination statute enacted by the United States Congress. The purpose of the Act is to prohibit discrimination and to assure that disabled students have educational opportunities and benefits equal to those provided to nondisabled students. An eligible student under Section 504 is a student who has record of having, or is regarded as having, a physical or mental impairment which substantially limits a major life activity.

- School Psychologist (New Referrals)
- Guidance Counselor
- General Education Teacher
- The Student (15 and older)
- School Physician (if requested by the parent/guardian)
- Related Service Providers (if applicable)
- Speech/Language Pathologist
- Occupational Therapist
- Physical Therapist
- Behavioral Specialist
- Visual Therapist
- Parent/Guardian may invite other individuals to attend Child’s Committee on 504 Meeting
- Please notify the Special Services Department, prior to the Committee on 504 Meeting, as to who will be attending the 504 Meeting in addition to the 504 Committee Members.
