West Vigo Middle School is very excited to announce that the Vigo County School Corporation High and Middle School Athletic Departments have partnered with FinalForms, an online forms, and data management service. FinalForms allows you to complete and sign athletic participation forms for your student-athlete(s). The most exciting news is that FinalForms saves data from season to season and year to year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible for each of your student-athlete(s), saving you time. You may review your data at any time to verify it is current. However, you must sign your forms annually and after any update.
We ask ALL athletes’ parents to use FinalForms in place of our physical paper packets. You must complete registration prior to your student-athlete practicing with a team for the 2022-23 school year. **If you have already completed and turned in the packet, we will update/upload the actual signed physical once you’ve initiated your account. You will only have to complete your student’s registration one time as the information rolls over to the following year. Once the registration is complete, your student-athlete’s name will be highlighted in green. The name appears with a red highlight if information or signatures are missing. We apologize for the additional step to documenting your student-athlete’s eligibility; it will greatly shorten the registration process in the long run.
