A mobile unit will be at Froebel. Please call the school nurse at 771-3533 or 934-5450 or email Jennifer.Howard@slps.org to schedule an appointment time. We will be completing physical exams and immunizations for enrollment. This includes preschool health requirements.

Consent forms can be printed from home or paper copies available at the school.
