You must complete both the parent/guardian permission for AND the modified sports health history form.

Instructions:

• Navigate to: https://app.operoo.com/users/sign_in
• If needed:
  • Click on “forgot password” – reset your password. (Your email address will be the same one you use for eSD Parent Portal)
  • Once you reset your password and log in, you will see a dashboard with all of your uncompleted forms. 
• Open the “Modified Sports Parent/Guardian Permission Form” (for each player)
• Once you complete that form, refresh the page, and you will see the “Health History for Modified Sports”
  • You will only see this form once you complete the parent permission form.
  • Complete this form for each player.
