NYS Requirements

Beginning September 2018, NYS requires physicals in grades PK or K, 1, 3, 5, 7, 9, and 11 and all NEW ENTRANTS to the district. They are also required ANNUALLY to participate in interscholastic sports and AS NEEDED for working papers, AND REQUIRED for an evaluation process for The Committee on Special Education or Committee on Pre-School Special Education. The health examination must be provided within 30 days of attendance of the required grade entry and must be performed by a physician, physician's assistant, or nurse practitioner licensed in NYS or meeting NYS requirements. If a physical is not submitted, a physical with the school doctor will be performed. A dental health record is requested at the time a physical is required as well. All forms need to be submitted no later than October 1st.

Beginning January 2021, the mandated form below will be the only physical form allowed.
