New Students are required to UPLOAD TWO proofs of residency.

You will need a to scan or take a picture of the required proofs and save those documents to your computer, phone or tablet. You will need these files to complete the registration process.

• Military Housing Letter- (only one needed)
• Lease/Rental Agreement
• Current MLGW bill
• Municipal water Bill
• Driver’s License or State ID
• Public assistance/government benefits form
• Real estate tax receipt
• Mortgage statement or deed
• Approved Shared Residency form (if Applicable)

In the addition of the above, NEW STUDENTS MUST provide the following:

• Report Card and/or transcript and withdrawal grades
• Discipline and attendance records
• Certified Birth Certificate
• Proof of Immunization record on Tennessee School form (out of state students have 10 days to transfer to TN form)
• Custody or Guardianship Legal Agreement (if Applicable)

• Carta de vivienda militar- (solo se necesita una)
• Contrato de arrendamiento/alquiler
• Factura actual de MLGW
• Factura municipal de agua
• Licencia de conducir o identificación estatal
• Formulario de asistencia pública/beneficios del gobierno
• Recibo del impuesto inmobiliario
• Declaración o escritura de hipoteca
• Formulario de residencia compartida (si corresponde)

Además de lo anterior, LOS ALUMNOS NUEVOS DEBEN proporcionar lo siguiente:

• Boleta de calificaciones y/o expediente académico y notas de retiro
• Registros de disciplina y asistencia
• Acta de Nacimiento Certificada
• Prueba del registro de vacunas en el formulario de Tennessee (los estudiantes de fuera del estado tienen 10 días para transferirse al formulario TN)
•  Acuerdo legal de custodia o tutela (si corresponde)
