Special Education Evaluation
A parent or guardian has the right to request a special education evaluation at any time. It is best to submit this request in writing to your school’s principal. If your child is pre-school age and not yet enrolled in school, send the letter to the district’s special education director. For more information, visit the Specialized Learning page.

504 Information
Due to updates to The Americans with Disabilities Act, your child may be eligible under Section 504. The purpose of identification under Section 504 is to prohibit discrimination and assure that students with disabilities have educational opportunities and benefits equal to those provided to students without disabilities.
Section 504 covers qualified students with disabilities who attend schools receiving Federal financial assistance. To be protected under Section 504, a student must be determined to:
- have a physical or mental impairment that substantially limits one or more major life activities; or
- have a record of such an impairment; or
- be regarded as having such an impairment. Section 504 requires that school districts provide a free appropriate public education (FAPE) to qualified students in their jurisdictions who have a physical or mental impairment that substantially limits one or more major life activities. Some examples of major life activities are seeing, walking, hearing, speaking, breathing, eating, concentrating, and also include limitations of major bodily functions and/or body systems (i.e., immune, digestive, bowel, respiratory, brain/neurological, circulatory, bladder, and endocrine.)
NOTE: If you would like to have more information regarding Section 504 evaluation due to any of the identified medical/health conditions, please contact your campus administration. Not all medical/health conditions require Section 504 evaluation and will be considered based upon the individual need.

Attendance and Truancy
Texas Education Code
Attendance for Credit - Sec. 25.092. MINIMUM ATTENDANCE FOR CLASS CREDIT OR FINAL GRADE. (a) Except as provided by this section, a student in any grade level from kindergarten through grade 12 may not be given credit or a final grade for a class unless the student is in attendance for at least 90 percent of the days the class is offered.
Truancy - 25.095(a) A school district or open-enrollment charter school shall notify a student's parent in writing at the beginning of the school year that if the student is absent from school on 10 or more days or parts of days within a six-month period in the same school year :(1) the student's parent is subject to prosecution under Section 25.093; and(2) the student is subject to referral to a truancy court for truant conduct under Section 65.003(a), Family Code.
A parent must provide a written and signed explanation for any absence upon the student’s arrival or within two days of the student's return to school.

Student Insurance
Under State Law, school districts are not liable for accidents which occur in schools. It is important to understand the school district is not responsible for medical payments or bills for your child. If your child is injured during any school, athletic or UIL sponsored activity, all medical charges are the responsibility of the parent/guardian. MISD has purchased a supplemental accident policy which allows you as a parent the opportunity to purchase student accident insurance. Click here for more information.

Emergency Administration of Epi Auto-Injectors and Respiratory Distress Medications
Notice to Parent of Emergency Administration of Unassigned Epinephrine Auto-Injectors and Administration of Medication for Respiratory Distress
In accordance with state law, Midlothian Independent School District has adopted a policy to allow authorized school personnel who have been adequately trained on the maintenance, administration, and disposal of unassigned epinephrine auto-injectors and medication for respiratory distress to a person who is reasonably believed to be experiencing an anaphylactic reaction or experiencing symptoms of respiratory distress. Authorized and trained individuals may administer an epinephrine auto-injector at any time to a person experiencing anaphylaxis on a school campus or may administer medication for respiratory distress at any time to a person experiencing symptoms of respiratory distress on a school campus.

Human Sexuality Instruction
*I understand Human Sexuality Instruction is required by TEA (Texas Education Agency) for students in Grades 5-12. I will read the MISD Human Sexuality Instruction notification when sent by my child's school prior to the delivery of the instruction and will contact my child's school if I have any questions.
Drug Prevention Program
Substance abuse prevention and intervention measures lead to healthy growth and development of youth. Identifying protective factors and effective coping methods to handle stressors play a critical role in building resilience. MISD offers prevention programming through community partnerships with AIM for Success and REACH Council. Both organizations assist with educational programming for youth on the risks of substance use and its effects on health and wellness. Random Drug Testing Program
The District may conduct random drug testing of any student in grades 9-12 who chooses to participate in school-sponsored extracurricular activities or requests to park a vehicle on school property. Review Board Policy FNF (Local).
*I understand that if I answer "NO" that my student will not be allowed to participate in any extracurricular activities nor will they be allowed to park a vehicle on school property.
Library Materials
Midlothian ISD acknowledges that parents and guardians are the primary decision makers regarding their student’s access to library material. Students are afforded the opportunity to self-select library materials as part of literacy development and the library program. The Midlothian ISD online library catalogs can provide a description of books as well as age ratings for books. District staff may assist a student in selecting library material; however, the ultimate determination of appropriateness remains with the student and parent or guardian. Parents and guardians are encouraged to communicate with the campus librarian and their child’s teacher about special considerations regarding library materials self-selected by their student. In accordance with state law and administrative procedures, parents or guardians may select alternative library materials for their student.
Information regarding the school libraries and book selection procedures can be found linked on the district website on the Library Services webpage. From the district webpage, select Departments and then Library Services.
Parents can access MISD online library catalogs from any campus library webpage or from the district Library Services webpage. Using their child’s login, parents can access Destiny for physical books and Sora for digital books to see what items have been checked out by their child. Digital resources can also be accessed by logging into the child’s account.

Stadium Rules and Clear Bag Policy
We wish to create an environment in which families with children of all ages may enjoy MISD events. We also want to ensure the safety of our guests by implementing the CLEAR BAG policy and venue guidelines found here, along with stadium student guidelines found here.
These guidelines are reviewed annually and updated as needed.
Field Trip Transportation
*I understand that transportation may be provided by MISD buses for school sponsored field trips.
