Lemont-Bromberek CSD113A Health Services
Each District 113A health office is staffed by a Registered Nurse. The nurses provide emergency care to students who are injured at school or during school-related activities. The nurses also collaborate with parents to address the needs of chronically ill children so that the children can remain in school. Health office procedures and requirements are regulated by Illinois School Code and the Illinois Department of Public Health. Physical/Dental/Eye Examinations and Immunizations
Physical, dental and eye examinations and immunizations, prescribed by State Law, are required of students in the categories below. Parents are requested to have their children examined by a licensed practitioner and to return physical, dental and eye examination forms and immunizations records to school before the school year begins. All physical exams and immunizations must be filed by October 15 of the school year, or by district policy, the student will be excluded from school. Forms can be found on the tabs to the left.
Early Childhood/PreK: Physical examination; immunizations (including varicella)
Kindergarten: Physical, dental and eye examinations; immunizations (including varicella)
Grade 2: Dental examination
Grade 6: Physical and dental examinations; state-required immunizations
Proof of a physical examination and immunizations are also required for 1st Grade students who have not attended District 113A's Kindergarten and students new to the District (transfer records will be considered).
A sports physical is required for any student participating on an athletic team. Sports physicals are “good” for 395 days from the date of exam. If the physical expires at any time during the sports season a new physical will be required.
