Butler Creek Elementary!!
To volunteer at Butler Creek you will need to complete the Vaccination Verification Form. Please follow this link:
School Volunteer COVID-19 Vaccination Verification Form
- You must include a valid email address.
- You will be required to upload an image of your vaccination card or other documentation of the COVID-19 vaccination. (Proof of a COVID-19 booster shot is not required at this time)
Once we receive the verification that your vaccination is up-to-date, you will need to complete a Criminal History Verification form. Contact the Butler Creek Office to obtain a copy of this form.
Once you complete the Criminal History form, return it in person with the $5.00 cash fee.
Butler Creek staff will then submit your application. Please be aware that due to the high demand, the waiting period for background clearances is a two week minimum.
