It is vital that the Health Office be notified of changes in parent's/guardian's phone number or address. There are times when a parent needs to be contacted when a child is ill or injured. This is very difficult when we have outdated information. The new information may be sent in by note or fax to (585) 242-5156.
It is also vital to have your cell phone and pager numbers in our records.
