Home Language Survey & Language Translation:
- Each time a student enrolls you are required to complete a new Home Language Survey. The link below takes you to OSPI's (Office of Superintendent of Public Instruction) website to print a form in a variety of languages.
Choose one of these options to get the completed form to the school:
1) Complete, Save, and Attach the form to the online registration.
2) Bring completed forms to your student's school.
3) Forms are available at the school if you do not have a printer.
