Immunizations - WA State

A current Washington State Certificate of Immunization Status (CIS) is required for enrollment in school. To obtain a CIS, you may do one of two ways:
- Visit the WA Department of Health web site and sign up for the Washington State Immunization System to download your child's immunization history and Certificate of Immunization form and either upload it during pre-registration or bring it to your school. Note: Your child's health care provider will need to be a participant in this program for your child's immunization information to be available for download.

Complete a Certificate of Immunization Status form, write in the immunization history, and either upload it during pre-registration OR bring it to your school.
