Immunizations - Out of State
To make sure your student enrolls with a complete immunization record, you will need to have a Certificate of Immunization Status form (also available in other languages).
You must download the Certificate of Immunization Status form and then write in the immunization history. Then upload the CIS to the online pre-registration OR bring the printed form to your neighborhood school after completing the online Pre-Registration. Thank you.
