Parent "No Access" Campus Library Request

This form allows parents or guardians to restrict access to specific books for their child and will not permit them to check them out or read them from the school library.

Before completing this form, please follow these steps:

  1. Check the school's library catalog to confirm that the book is available in the library collection.

  2. If the book is available, complete this form to notify the school librarian and your child’s teacher.

Este formulario permite a los padres o tutores restringir el acceso a libros específicos para su hijo(a). 

Antes de completar este formulario, por favor siga estos pasos:

  1. Revise el catálogo de la biblioteca escolar de su hijo(a) para confirmar que el libro está disponible.

  2. Si el libro está disponible, complete este formulario para notificar al bibliotecario escolar y al maestro(a) de su hijo(a).
