Enrollment Materials Checklist
The following items need to be returned when enrolling the student.
- Enrollment/Emergency Information
- Proof of Residency (Utility Bill, Rental/Lease Agreement) and driver's license
- Move-in affidavit requires a purchase agreement and driver's license.
- Live-with affidavit requires a driver's license from both parties and a utility bill from the property owner.
- This affidavit has to be renewed each year.
- Both affidavits are obtained at the NACS Corporation Office at 13119 Coldwater Rd.
- Residency Verification Form
- Birth Certificate
- Guardianship Papers (Joint Custody Papers)
- Former School Address
- Parent/Guardian Permission Signature Sheet
- Course Selection Sheet
- Race/Ethnicity Reporting
- Home Language Survey
- Personal Health History Form
- Athletic Transfer Form (All new students grades 10-12 or all students after school has started.)
- Device Protection Plan
