Consent for Disclosure of Personally Identifiable Information

Your consent, or the consent or your child if he or she has reached the age of majority, must be obtained before personally identifiable information is released to officials of participating agencies providing or paying for transition services. In addition, if your child attends a private school, your consent must be obtained before any personally identifiable information about your child is released between officials in the district where the private school is located and officials in the district where your child resides if you are not planning to enroll your child in your district of residence. Under FERPA (Family Educational Rights and Privacy Act) your consent is not required before personally identifiable information is released to officials of participating agencies for purposes of meeting a requirement of Part B of IDEA. For example, other schools to which your student is transferring.
