If you live outside our district please visit our district webpage and click on the PARENTS link followed by the OPEN ENROLLMENT link. Do not fill out Final Forms until you have been approved and contacted by the building secretary.
The following information is required for registration:
* Certified Birth Certificate (can be a copy)
* Current Immunization Record
* Proof of Residency (see below)
* Custody Paperwork (if applicable)
*Proof of Residency MUST be submitted. Please provide a copy of ONE of the following Per ORC 3314.11: A deed, mortgage, lease or rental agreement; Current home owner’s or renter’s insurance declaration page, or current real property tax bill; A utility bill issued within 90 days of enrollment; A paycheck of paystub issued within 90 days of enrollment; or the most current available bank statement issued that includes the address of the primary residence
