North Carolina Vaccine-Specific Requirements

The North Carolina General Statutes (G.S. 130A-152(a)) require immunizations for every child present in this state. Every parent, guardian or person in loco parentis is responsible for ensuring that their child(ren) receive required immunizations

Requirements for School Entry

• 5 doses Dtap
• 4 doses Polio
• 2 doses MMR
• 3 doses Hepatitis B
• 2 doses Varicella
• 4 doses Pneumococcal
• 1 booster doe Dtap (7th graders)
• 1 Meningococcal conjugate vaccine (MCV) (7th graders)
• 1 booster dose of Meningococcal conjugate vaccine (MCV) (12th graders)

 Kindergarten Health Assessment 

Kindergarten Health Assessments are required for all students entering Kindergarten for the first time. This is required by North Carolina State Law General Statue 130A-52. The form can be downloaded in English or Spanish by accessing the link or obtaining a form at your health care provider. It must be the Kindergarten Health Assessment Form that states North Carolina Health Assessment Transmittal Form (we cannot accept any other form). Please proofread the form before it is turned into the school to make sure that it is complete, signed and dated. If you have any questions regarding this form, please contact your health care provider or your school nurse. 

All Students That Are New To North Carolina Public Schools

North Carolina House Bill 13 states that every student who is new to NC Public Schools must have a health assessment completed within 30 days of enrolling. The health assessment must be completed using the North Carolina Health Assessment Transmittal Form. You must also submit a completed immunization record. If you have any questions concerning this please ask your school or the school nurse for help.

Do you have a 7th grader?

Then it is time to make sure their immunizations are up to date. Every 7th grader is required to have a TDAP and Meningococcal vaccine. 

Do you have a 12th grader? 

The second dose of the Meningococcal vaccine is a required booster for individuals entering the 12th grade or by 17 years of age, whichever comes first. If you entered the 7th grade before July 1, 2015 you are not required to receive the first dose. The booster dose does not apply to individuals who entered the 12th grade before August 1, 2020. If the first dose is administered on or after the 16th birthday, a booster dose is not required. Individuals born before January 1, 2003 shall not be required to receive meningococcal conjugate vaccine. 

Medication Administration

Granville County School Policy states that a medication/authorization form is required for all students to receive medication (prescription or over the counter) at school. It must be completed and signed by parent and the student's doctor.
