Calhoun's Athletic Trainers are once again happy to coordinate athletic/UIL physicals that will be required for athletic/band/cheer/drill team/ROTC programs next school year. The physicals will be conducted by physicians at Calhoun High School, 201 Sandcrab Blvd in Port Lavaca according to this schedule:
Monday, April 28: All BOY Athletes 5:30 p.m. to 7 p.m.
Tuesday, April 29: All GIRL Athletes 5:30 p.m. to 7 p.m.
* No check in will be allowed after 7 p.m. *
The check in location is at Calhoun High School, in the Gym 1 (game gym) hallway.
This is a one-time, annual event, and we appreciate our partnering physicians for their participation. Please note that this event is only for school athletic/UIL physicals required for UIL participation. No additional medical consultations or physicals will be available.
The cost for a physical is $20 cash only.
All girls/boys that are currently planning to participate and try out for athletic/band/cheer/drill team/ROTC programs at any level will need to have a physical regardless of age or grade level.
Please complete/bring the following to the physicals:
1) Physical form
English translation
Spanish translation
Hard copies available from coaches/sponsors/or CHS Training office

A physical form will need to be fully completed before the physical will be given. Hard copies of the physical forms are available from the middle school coaches or the CHS Training Room.
* Families may obtain physicals from their own doctor, if they choose, but are asked to use the official Calhoun physical form. *
