Absence Excuse Form

Good attendance is critical for success in school. We do understand students will be absent. When a student must miss school, a written excuse from a parent or guardian must be presented to the teacher within two school days of the student’s return after an absence. Written excuses submitted beyond two school days will be subject to principal approval. When we receive this email, you will be contacted to confirm. If the daytime number is different from our records, we will use the phone number from our records.
