Grades 1-8 Registration

Welcome to the Bolton Center School Registration page.  Parents wishing to register a student for grades 1-8 should contact Administrative Assistant,  Judy Ramsdell to make an appointment, (860-643-2411).  Please complete the following documentation, and bring this with you to your appointment. 

• Registration Packet
• Health Assessment Record
• Verification of student’s age (to include but not be limited to):
  • Photocopy of child’s birth certificate
  • Earlier school records
  • State issued identification documents
  • Parent’s affidavit or unsworn statement as to student’s age
  • Physician’s certificate verifying student’s age
  • Immunization Records 
• Proof of parent's residency (see Bolton Residency Requirements)
• Student Residency Affidavit (if applicable)
• Certificate of Residency (if applicable)
